
The repair form is specifically designed to meet JA standards. The envelope includes the JA logo, identifying a retailer as a member of Jewelers of America. Consumers must read, understand, and sign the envelope, underneath the disclaimer, before handing over their gemstones or jewelry for repair.
The disclaimer helps prevent miscommunication between JA members and their customers. The disclaimer on the JA-approved envelope (form model # JA-RE7) reads as follows:
The description and value of the article listed above are mine alone. I understand this store is not responsible for identification of the stones or jewelry listed above. Any gemstone in the submitted article may have been subjected to a possible undetectable enhancement process. I affirm that unless I have stated so above, I am unaware of any treatment or enhancement done to this gemstone. I understand that the store is not responsible for damage that is beyond the store’s control and attributable to unstable enhancement processes. I am also aware that there may have been other enhancements or treatments that I am unaware of done to the gemstone. I further understand this store is not responsible for loss, damage, and/or theft of my jewelry unless proven it was due to the store’s negligence. I further understand damage and/or loss strictly due to such negligence is limited to the actual cost of repair or replacement, not to exceed the value I listed above and, when no value is listed, it is agreed that the value is no greater than $75.00. I have read and understand this disclaimer of liability, its purpose being to induce this store to repair my jewelry and I agree to be bound by all of its terms and conditions.
These take-in repair envelopes can only be used by JA members in good standing.
